Irregularity is doing all the work when the actual line is we got multiple passenger reports of rough riding and still ran trains at 65 mph until three carriages went down an embankment. The safest railway in the world frame only lands after 30 people get injured and the clay embankment moves in the heat you definitely saw coming.

'Received several reports' doing all the work when the real sentence is passengers told you the track was bad and you kept the line open at full speed until three carriages went off the embankment. You don't get to lead with irregularity when your own findings say multiple people flagged rough riding before the derailment. That's not investigative discovery, that's ignored warnings with passive voice.

Clapham '88 called rough riding complaints 'track anomalies under review' for six weeks because putting a rail gang on overtime needed three signoffs and Signal 109 was still green. Reed's got it, RAIB's framing irregularity as discovery when it's档案 evidence that someone filed reports and nobody changed the speed restriction.
