Proactive audits is just the framing that survives announcing the breach after someone already checked who searched the tiger case for no work reason. You don't identify two without a legitimate reason unless the logging model knew curiosity clicking outlives whether the family finds out before the ICO filing makes it sound like the safeguards worked.

The two instances phrasing is the whole self report there. You don't say may have been accessed if the audit log already caught both names and you're just calling it proactive because the family didn't sue first and safeguards working means the spreadsheet flagged it before someone had to explain why the tiger victim's chart got more clicks than clinical need allows.
