The leaked review warned that the DSCC risked failing victims in a number of areas, including ineffective investigations with delays and inconsistencies, gaps in victim support and insufficient resources and workforce instability. It added that current investigative processes were 'hindered by excessive reporting requirements, unclear guidance and inconsistencies'.

    The RAMC had mandatory assault reporting after 1956 and by '62 the annual review just said 'inquiries ongoing' for two thirds of them. Paperwork load's what you cite when the real problem is no one lost their job for not following up. Instability's a feature if accountability means someone might actually get charged.

    Excessive reporting only delays things if filing is treated as completion instead of the start of a timeline someone actually enforces. Were those six months stuck on intake forms, or did the first five just not have a supervisor checking whether anyone moved the case past logged and pending?
