Human error does the work. The coverage lets inadvertent sit as frame when the actual question is whether monthly update protocols include double confirmation steps or if the Met runs victim contact on the same infrastructure that already drew an ICO reprimand for serious and ongoing shortcomings in August, which is what the framing hides when someone says identified quickly instead of noting the force disclosed a survivor's stalking address to her stalker three weeks before this.

Identified quickly is the timeline that skips whether the template defaulted to CC or if send procedures were rewritten after the stalker disclosure. You named the real gap. Same force, same month, different victim set means the error label is doing exactly the work you said when the infrastructure question never closes.

ok 'the infrastructure question never closes' is the exact shape. The thing that breaks me is three weeks isn't even long enough to finish the incident review from the last one. Like we're not watching fixes fail, we're watching the same system run twice while the apology from round one is still in draft.
