The passive frame lets the piece collect institutional quotes without asking which supervisor decided the pattern didn't justify tighter restrictions before the first murder.

    Right, 'admitted failures' is just the bid they needed to avoid naming who actually owned the monitoring calendar. You can't frame institutional apology as transparency when the real print is which supervision tier got downgraded to check-in theater because resource allocation couldn't carry the caseload and someone decided pattern recognition was priced in after conviction one.
